New Client Information & Forms
Pre-Appointment Preparation
If you care to complete your paperwork prior to coming into Capital Cat clinic choose from the following forms. If you have never been into the clinic you will need to download, print and fill out all three forms. If you have a new addition to your family and are currently a client you will just need to fill out the "New Patient Form". All exams and procedures are by appointment only, so please call to schedule your appointment.
More than one cat?
If you have more than one cat please fill out a "New Patient Form" for each cat. Note all files are in a PDF Format.
Submitting Forms
You may either bring in your paperwork at your scheduled appointment time
or you may fax your completed forms to our office...703-522-1092 fax.
Faxing your forms in will allow us to speed up the process and have you entered in our computerized record keeping system.
Day of Appointment
Please arrive 10 minutes before your scheduled appointment time so we can review any records from prior veterinarian clinics. For the safety of your cat and all clinic patients, cats must be in carriers or on appropriate feline harnesses. Thank you!
